The facility requested a location indicator change via correspondence dated June 6, 2023 but accessed June 7, 2023 in the a.m. The legal name is Sarah A. Reed Children's Center. The location indicator change is from Herr Hall to James N. Herr Hall. This change was made effective June 6, 2023. The facility advised that this is the location indicator name on their certificate of compliance during the 2023-2024 Attestation Project, thus they wanted to have consistency once it was brought to their attention.
Sarah A. Reed Children's Center James N. Herr Hall is in compliance with the Requirements of 42 CFR Part 483, Subpart G Requirements for Psychiatric Residential Treatment Facilities.
